Birth & Death Certificates

Computer downlink allows Scott County Health Department access to vital records at Missouri Department of Health in Jefferson City, Missouri.

Certified copies of Birth Certificates can be printed at our office while you wait.  Certificates are available for any birth occurring in the State of Missouri after 1920.

Certified copies of Death Certificates can be printed for any person whose date of death is after 1980.

Birth Certificates are $15.00 each.  Death Certificates are $14.00 for the first one, and $11.00 for each additional copy.  We accept cash, check, credit card, or money orders.

To request a birth or death certificate, print the following form, complete, and bring or mail to the address on the form along with payment per certificate fee.  Certificates are available immediately if you present the form in person.  Allow one to two weeks for certificates requested by mail.  

You Need

• Full name on certificate
• Date of birth or death
• Place of birth or death
• Full name of Father and Mother, including Mother's maiden name
• Your relationship to the individual named on the record
• The purpose for the certificate request
